Children’s Back-to-School Community Health Fair
Join Children’s Healthcare of Atlanta for our fist Back-to-School Community Health Fair. We are excited to be working with local youth-serving organizations to get kids ready to go back to school. Join us for health screenings, education, fun kid's activities and more.

Health screenings
School-required health screenings and immunizations
If your child hasn’t seen a healthcare provider for the school-required health screenings and immunizations, schedule an appointment* with the Ronald McDonald Care Mobile® to receive the following paperwork:
- GA Public School Form 3300: Hearing, Nutrition, Vision and Dental
- GA Public School Form 3231: Required Vaccines
- GHSA Sports Physical Form
Schedule Screening
Registration prior to the health fair is strongly encouraged. There are a limited number of appointments and walk-ins available.
*Insurance is accepted but not required.
Eye exams
Vision to Learn is offering no-cost eye exams and prescription glasses (when needed) for students from pre-K through 12th grade. Schedule an appointment with the Vision to Learn van.

Health education
Visit the different tables at the event for healthy habits resources, injury prevention tips, asthma education and more.
Connecting to resources
We will also be welcoming local youth-serving organizations, such as Sheltering Arms, Vision to Learn, Atlanta United, Soccer in the Streets, the YMCA, Boys and Girls Club of America, and SafeKids.
Kid's activities
We'll have a DJ from our beloved Challenge Team to make sure everyone is having a good time with music, dancing, raffles and other fun activities.
