What is the Strong4Life Clinic?
The Strong4Life Clinic is a multidisciplinary program dedicated to obesity treatment. The clinic offers lifestyle, medications and bariatric surgery treatment options.
At the Strong4Life Clinic, patients are placed on a treatment track which includes medical provider, dietitian and exercise physiologist visits. All tracks include teaching kitchen and activity experiences, and patients may also see the clinic psychologist if needed.
Strong4Life Clinic referral criteria
- Children and adolescents, ages 2 to 20 years old
- Diagnosis of Class 3 obesity (BMI >140th of the 95th percentile)
- Diagnosis of Class 2 (BMI >120th of the 95th percentile) with obesity with a weight-related comorbidity (e.g., prediabetes, dyslipidemia or steatotic liver disease, obstructive sleep apnea, PCOS, HTN)
- Interest in bariatric surgery (must meet surgery qualifications)
Strong4Life Bariatric Program referral criteria
- Patients with a BMI greater than 40
- Patients with a BMI greater than 140th of the 95th percentile
- Patients with a BMI greater than 120th of the 95th percentile, with at least 1 significant weight-related comorbidity
Strong4Life Clinic referral process
To schedule an appointment with the Strong4Life Clinic, we need the following:
- A referral from the child's physician
- A new patient packet filled out by the caregiver

Referral from your child's doctor
Please ask your child's doctor to fax your referral to 404-785-9111. It should include:
- Most recent clinic notes
- Most recent lab work results
- Growth charts
- Demographics
- Contact information

Note: Due to high demand, scheduling priority is given to the following groups:
- Patients with severe Class 3 obesity, with multiple comorbid conditions
- Children younger than 6 years old
- Adolescents pursuing bariatric surgery

For your patients who do not meet the criteria for the Strong4Life Clinic but are interested in weight management counseling, view a list of registered dietitians who have participated in Strong4Life training.
